feat: add SSE real-time updates for MiniApp dashboard

Push plan, session, and skills state changes to the MiniApp via
Server-Sent Events instead of requiring manual refresh. A lightweight
StateNotifier fans out notifications from AgentLoop's three mutation
points (post-LLM state machine, /plan commands, plan creation) to all
connected SSE clients, with JSON diff dedup to avoid redundant writes.
