refactor: reduce runLLMIteration complexity from 94 to 19
Extract callLLMWithRetry, cleanLLMResponse, executeToolCalls, buildAssistantMessage, publishToolMedia, forceTextResponse as separate methods. Add no-op defaults to iterationHooks eliminating all nil checks in the main loop. Cyclomatic complexity: 145 → 94 → 19. Branching statements: 109 → 59 → 14.
