fix(exec): implement per-command timeout, relax overly strict safety guard, and add system bin dir whitelist

Resolves YouTube audio download failures caused by multiple systemic issues:

1. Per-command timeout: executeRun() now reads the declared but unimplemented
   `timeout` parameter. LLM can pass timeout=600 for downloads. Fractional
   seconds preserved via float64 multiplication. timeout=0 falls back to global
   default instead of disabling protection.

2. Relaxed deny patterns: removed 6 overly broad regex patterns that blocked
   legitimate operations — command substitution $(), variable expansion ${},
   backticks, heredoc, $(which ...), and `pip install --user`. Safety-critical
   rules (curl|sh, rm -rf, sudo, eval, etc.) remain intact.

3. System bin dir whitelist: paths under /usr/bin, /usr/local/bin,
   /opt/homebrew/bin, etc. now bypass the restrictToWorkspace check, allowing
   agents to reference system tools like yt-dlp and ffmpeg.

4. Command not found hint: when an executable is not found (including full-path
   commands on macOS), the output includes a hint suggesting `which` or
   `command -v` to verify the correct path — reducing LLM path hallucination.
