docs: add memory optimization review findings to CLAUDE.md

Parallel review of ~50 Go source files across agent/, providers/,
tools/, channels/, config/, skills/, utils/ packages. Findings
categorized by: string concat, slice pre-alloc, byte/string conversion,
JSON hot paths, struct copies, sync.Pool candidates, and LRU algorithm.
